2) Write in BLOCK LETTERS in the spaces provided or tick in the appropriate box.

3) Supplying of false information is a criminal offence and will result in automatic disqualification and rejection of the application. Further action may be taken.

4) Attach Certified copies of each Academic certificates, BIRTH certificate and ID.

5) Submit the completed application form to the Admissions Office, Mlezu Agricultural College or online through the college website.
